Patient Care Coordinator

Responsibilities

• Guide patients through care pathways with compassion and precision
• Coordinate appointments to maximize provider utilization
• Respond to patient inquiries via phone, email, and other channels
• Maintain accurate patient records in DrChrono and Salesforce
• Protect patient rights and maintain HIPAA confidentiality
• Support billing processes including copay and cash pay collection
• Participate in daily huddles and weekly team meetings
• Maintain compliant documentation in the EMR

Requirements

• High school diploma or equivalent
• Minimum 2 years of experience in healthcare coordination or medical office administration
• Experience with scheduling systems or EMRs
• Strong organizational and interpersonal skills
• Ability to reliably commute to the Westmont, IL clinic

Preferred Qualifications

• Background in behavioral health or interventional treatment settings
• Familiarity with DrChrono EMR
• Experience coordinating care across interdisciplinary healthcare teams
• Knowledge of insurance verification and medical billing
