Patient Care Coordinator

Responsibilities

• Complete individualized patient care plans and perform care management using the electronic medical record system
• Provide care coordination and support to patients via frequent contact to improve outcomes
• Document all client communications accurately
• Communicate patient emergent needs to Primary Care Clinicians and ensure care plans are complete
• Keep Team Supervisor informed of care plan issues and client concerns
• Collaborate with cross-organizational teams to meet patient and family needs
• Use technology including secure email, telephone, and electronic medical records
• Adhere to documentation protocols for daily logs and escalation of issues
• Operate office equipment including computers, telephones, and copy machines

Requirements

• Comfortable speaking on the phone for extended periods
• Compassionate and empathetic with exceptional customer service skills
• Accurate note-taking and timely documentation abilities
• Ability to multitask and remain organized in a fast-paced environment
• Proficient in Microsoft Office programs including Outlook
• Knowledge of basic healthcare terms, conditions, and care principles
• Must work full-time Monday through Friday, 8:30am–5:00pm on-site in Marlton, NJ
• Able to pass a drug screen and background check
