Patient Care Coordinator

Responsibilities

• Complete individualized patient care plans and perform care management using the electronic medical record system
• Contact patients frequently to coordinate care, provide support, and manage compliance with care programs
• Document all client communications accurately and promptly
• Communicate patient emergent needs and life-threatening episodes to the Primary Care Clinician
• Keep the Team Supervisor informed of care plan issues and process updates
• Collaborate with cross-organizational teams to meet patient, family, and partner facility needs
• Use technology including secure email, telephone, and electronic medical records effectively
• Support caregivers and families through scheduling and day-to-day assistance

Requirements

• Comfortable speaking on the phone for large portions of the day
• Compassionate and empathetic with a commitment to exceptional customer service
• Strong note-taking and timely documentation abilities
• Ability to multitask across patients and workstreams while staying organized
• Capacity to thrive in a fast-paced environment
• Full-time availability Monday through Friday during daytime hours
• On-site presence required in Hackensack, NJ
• Proficiency with computers, Outlook, and Microsoft Office programs
• Knowledge of basic healthcare terms, conditions, roles, and care principles
• Ability to pass a drug screen and background check
