Patient Care Coordinator

Responsibilities

• Coordinate and manage patient care activities through the EMR system
• Support clinical workflows including referrals, laboratory orders, and medication refills
• Maintain communication with patients, providers, and families to resolve barriers to care
• Document all patient interactions accurately in the EMR
• Manage multiple priorities in a fast-paced environment using office technology

Requirements

• Active Medical Assistant (MA/CMA) certification or comparable clinical credential
• Minimum of one year of clinical experience
• Strong knowledge of healthcare terminology, diagnoses, and medications
• Proficiency in Microsoft Office and EMR systems
• Ability to work on-site in Waco, Texas, Monday through Friday, 8:00am to 5:00pm CST
• Legal authorization to work in the United States

Preferred Qualifications

• Experience supporting providers or working in a medical office/family practice setting
• Experience coordinating patient care
