About the role
Responsibilities
- Coordinate patient-centered care activities using the EMR system
- Support providers and clinical teams through accurate documentation and follow-up tracking
- Serve as a communication link between providers and internal care teams
- Communicate patient needs and urgent concerns to the clinical team
- Assist with care transitions, referrals, and coordination with external partners
- Maintain accurate documentation in accordance with established workflows and protocols
Requirements
- Active clinical certification or licensure (PCT, CNA, MA/CMA, LPN, or RN)
- Minimum 1 year of experience in a clinical or care coordination role
- Proficiency with Microsoft 365 (Outlook, Word, Excel)
- Strong knowledge of healthcare terminology
- Ability to work onsite in Voorhees, NJ, Monday through Friday, 8:00am - 4:30pm EST
- Ability to pass a background check and drug screen
