About the role

Responsibilities

  • Provide case management and support to individuals and families navigating social adjustment challenges through the Pathway of Hope program.
  • Conduct client intake, eligibility assessments, and develop individualized action plans and goal sets.
  • Perform ongoing monitoring and follow-up assessments at 30, 60, 90-day, and 12-month intervals.
  • Coordinate resources and provide referrals for housing, health benefits, and financial support.
  • Maintain accurate client data and documentation within the WellSky Community Services system.
  • Network with community partners and outside agencies to build a repository of local resources.
  • Support the spiritual well-being of clients by connecting them with local ministry leaders.

Requirements

  • Bachelor of Arts in Social Work preferred, or 2-3 years of Case Management experience.
  • Minimum of one year of experience working with low-income or homeless families.
  • Valid driver's license.
  • Proficiency in Microsoft Word, Excel, and PowerPoint.
  • Ability to undergo applicable screening and background checks.
  • Ability to work with diverse populations in a trauma-informed, sensitive, and non-judgmental manner.

Preferred Qualifications

  • Knowledge of Strengths-Based Perspective, Motivational Interviewing, and the Stages of Change Model.
  • Basic understanding of mental health disorders and the unique issues surrounding homelessness.
