About the role
Responsibilities
- Provide direct case management using the Pathway of Hope framework, including intake, assessment, goal planning, and follow-up.
- Maintain a caseload of 6-12 participants, helping them develop action plans to foster independence and break cycles of dependency.
- Coordinate with ministry units, community partners, and volunteers to provide holistic support to clients.
- Complete and maintain accurate client records, assessments, and daily statistics while ensuring strict confidentiality.
- Build rapport with at-risk and homeless populations and connect them to essential community resources and referral services.
- Conduct home visits and provide accompaniment services as required to support client progress.
Requirements
- Post-secondary degree or diploma in Social Work or a related Human Services field.
- Previous experience working in social services or mental health settings.
- Valid Driver's License and a clear Driver's Abstract (personal vehicle may be required).
- Current First Aid/CPR certification.
- Ability to pass a Criminal Record Check and Child Abuse Registry clearance.
- Proficiency in Microsoft Office (Word, Excel, PowerPoint, Outlook).
- Strong interpersonal, communication, and organizational skills.
Preferred Qualifications
- Minimum of 2 years of experience working with inner-city agencies and clients.
- Experience in program facilitation, administrative duties, and program development.
- Knowledge of substance abuse, crisis intervention, and money management/trusteeship programs.
