Responsibilities

  • Drive measurable referral growth within assigned territory through strategic partner engagement
  • Serve as the face of Forge Health/KPC in the market to expand referral relationships
  • Execute strategic market activation plans aligned with organizational growth goals
  • Identify and prioritize high-opportunity referral sources and implement targeted outreach initiatives
  • Educate referral partners on appropriate level-of-care placement, specifically IOP and Psychiatric Services
  • Participate in community events, county meetings, and behavioral health coalitions
  • Collaborate cross-functionally with Marketing, Clinical, Intake, and Operations teams
  • Maintain CRM documentation and share market insights and referral trends

Requirements

  • 2+ years of experience in behavioral health, healthcare outreach, provider relations, or business development
  • Working knowledge of behavioral healthcare models and referral processes
  • Strong relationship-building skills with clinicians and healthcare organizations
  • Familiarity with CRM systems and performance tracking
  • Valid state license and reliable access to a vehicle
  • Ability to work flexible hours, including occasional evenings and weekends

Preferred Qualifications

  • Existing behavioral health network connections within the Long Island territory
