Palliative Care Pharmacist

Responsibilities

• Deliver high-quality palliative care to patients ensuring comfort and dignity
• Work with a multidisciplinary team to develop and implement individualized care plans
• Provide emotional support to patients and their families
• Coordinate with other healthcare providers to ensure comprehensive care
• Maintain accurate patient records
• Participate in ongoing training and professional development

Requirements

• Up to date GPhC registration
• Relevant Pharmacy qualification
• Hospital experience as a pharmacist

Preferred Qualifications

• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
