Palliative Care Nurse Practitioner

Responsibilities

• Assess and manage pain and complex symptoms in outpatient settings
• Lead goals-of-care and advance care planning conversations
• Support hospice readiness and facilitate referrals when appropriate
• Collaborate with physicians, specialists, home health, hospice teams, and community partners
• Participate in interdisciplinary team meetings and care conferences
• Coordinate transitions across outpatient, hospital, home health, and hospice settings
• Serve as a clinical resource and advocate for patients and families

Requirements

• Active Nurse Practitioner license
• National board certification
• Current state licensure and DEA registration
• Experience in palliative care and complex symptom management
