About the role
Responsibilities
- Engage with individuals and families in the community to provide case management for social needs, including safe shelter, substance use disorder, and mental health resources.
- Work closely with police officers, city staff, community mental health agencies, and school departments to provide follow-up and referral services.
- Facilitate the department’s Adverse Childhood Experiences Response Team (ACERT) program, including outreach, deployments, and data collection.
- Develop and maintain an extensive network of community partnerships and knowledge of local service agencies.
- Monitor department databases for service-eligible incidents such as domestic disturbances, drug overdoses, and mental health crises to provide appropriate assistance.
- Coordinate and schedule community deployments and conduct timely follow-ups to ensure service delivery.
- Maintain accurate records and databases in accordance with agency, state, and federal confidentiality standards.
Requirements
- Bachelor's degree in Social Work, Criminal Justice, Psychology, or a related social science field.
- Minimum of three years of related professional experience.
- Strong communication skills, both verbal and written, with the ability to build rapport and trust with diverse populations.
- Proficiency with computers and modern office methods.
- Ability to handle emotionally charged situations and work effectively with individuals experiencing trauma.
Preferred Qualifications
- Prior experience with home visiting.
- Experience working collaboratively with public safety organizations.
