Outpatient Transition Social Worker - Stroke

Responsibilities

• Conduct psychosocial assessments of patients and their significant others regarding social, emotional, and cognitive capabilities related to illness.
• Develop and implement intervention plans to address identified needs and problems discovered during assessments.
• Collaborate with a multidisciplinary healthcare team to establish and ensure appropriate discharge planning.
• Utilize knowledge of community-based services and resources to meet the patient's continuum of care needs.
• Advocate on behalf of patients and families to help them obtain necessary services.
• Interact with co-workers, visitors, and staff in alignment with organizational values.

Requirements

• Proven expertise in psychosocial assessment and discharge planning.
• Strong knowledge of community resources and services.
• Ability to work collaboratively within a multidisciplinary healthcare environment.

Preferred Qualifications

• Previous experience working with stroke patients or in a stroke-specific clinical setting.
