One Point Patient Care Specialist

Responsibilities

• Serve as the primary point of contact for assigned physicians and patients to ensure seamless care coordination.
• Conduct welcome calls with new patients to gather information, obtain consent, and introduce program offerings.
• Manage the patient journey from enrollment through therapy initiation, including medication delivery and self-injection training coordination.
• Act as a patient advocate by exploring funding options and assisting with reimbursement processes.
• Maintain expert-level knowledge of assigned molecules and Canadian public/private reimbursement mechanisms.
• Ensure accurate and timely documentation of all patient interactions within the CRM system.
• Promptly report all Adverse Events (AE) and Product Technical Complaints (PTC) in accordance with company policy.

Requirements

• Post-secondary degree in healthcare, life sciences, nursing, pharmacy, or a related field.
• 3–5 years of experience in patient support programs, case management, or a specialty pharmacy setting.
• Strong understanding of Canadian healthcare reimbursement processes (public and private sectors).
• Proficiency in CRM systems and Microsoft Office Suite.
• Excellent communication, organizational, and problem-solving skills.

Preferred Qualifications

• Bilingualism in English and French.
• Experience using Kroll CRM.
• Proficiency in Power BI.
