About the role
Responsibilities
- Provide comprehensive medication management including reconciliation, dose optimization, and adherence support
- Partner with physicians and interdisciplinary teams to order individualized care plans under a CPA
- Monitor therapy effectiveness and safety to identify optimization opportunities
- Support quality-improvement and REMS initiatives
- Assist operations with complex prior authorizations and workflow improvements
Requirements
- Doctor of Pharmacy (PharmD)
- Licensed pharmacist in California
- Completion of a PGY2 Oncology Residency OR completion of PGY1 Ambulatory Care Residency with 3 years of oncology experience OR 3 years of oncology clinical pharmacy experience including 1 year under a CPA
- Proficiency in Microsoft Office tools
