Oncology Care Coordinator

Atlas is built for oncologists by oncologists. We provide practices with the staff, contracts, and toolkit they need to deliver truly whole person care to their cancer patients through a value-based approach to care delivery.

Responsibilities

• Serve as a primary point of contact for patients and families to navigate appointments and treatment
• Connect patients to educational resources regarding diagnosis and treatment
• Screen for potential barriers to care including logistical, financial, cultural, and psychosocial needs
• Assist with scheduling and follow-up appointments across the oncology continuum
• Support connection to support groups, counseling, and social work services
• Document patient interactions and support activities in the EMR
• Work closely with RN Care Coordinators and Clinical Teams to ensure seamless care
• Support in phlebotomy and vaccinations as needed

Requirements

• Bachelor's degree
• Strong communication and interpersonal skills
• Proficiency in EMR use and task tracking systems
• Experience in phlebotomy or Medical Assistant License
• Comfort working in a start-up or growing organization

Preferred Qualifications

• Bachelor's degree in social work, Public Health, Nursing, or Health Education
• Oncology Patient Navigator Certification
• Experience in patient navigation, care coordination, or oncology
• Experience addressing social determinants of health
• Experience advocating for patient financial, transportation, or housing assistance
