Occupational Therapist

Responsibilities

• Deliver occupational therapy interventions within an intermediate care service, supporting patients in their own homes or community settings
• Complete functional assessments and develop individualized rehabilitation plans
• Work collaboratively with the multidisciplinary team to ensure safe and timely discharges
• Promote independence and optimize patient outcomes through evidence-based practice

Requirements

• HCPC registration as an Occupational Therapist
• Experience in community rehabilitation or intermediate care
• Strong understanding of discharge pathways and admission avoidance strategies
• Excellent communication and teamwork skills
• Valid driving licence and access to a car
