Occupational Therapist

Responsibilities

• Deliver occupational therapy interventions within an unplanned care service, focusing on admission avoidance and discharge facilitation
• Complete functional assessments and provide equipment recommendations to promote independence
• Work collaboratively with the multidisciplinary team to ensure patient-centered care
• Respond promptly to urgent referrals and manage a varied caseload effectively

Requirements

• HCPC registration as an Occupational Therapist
• Experience in acute or community settings, ideally within unplanned care or rapid response services
• Strong understanding of discharge pathways and admission avoidance strategies
• Excellent communication and teamwork skills
• Valid driving licence and access to a car
