Occupational Therapist

Responsibilities

• Support discharge planning, prevent admissions, and manage frailty cases
• Provide interventions for patients with falls risk, neuro, and orthopaedic needs
• Work collaboratively with the multidisciplinary team to deliver patient-centered care
• Promote independence and functional recovery through evidence-based practice

Requirements

• HCPC registration as an Occupational Therapist
• Proven experience in acute and/or community settings
• Valid driving licence and access to a car
• Strong understanding of discharge pathways and admission avoidance
• Excellent communication and teamwork skills
