About the role
Responsibilities
- Participate as a full member of the multidisciplinary team in assessing, planning, implementing, and evaluating individual treatment and rehabilitation plans
- Liaise with relevant agencies, relatives, and carers with empathy
- Provide information, support, and advice regarding treatment plans
- Liaise with outside organisations for equipment provision, such as wheelchair services
- Provide advice and support to staff by teaching relevant techniques to safely implement treatment plans
Requirements
- HCPC registration
- Experience utilizing an evidence-based, patient-centred approach to assess, plan, implement, and evaluate treatment plans
