About the role

Responsibilities

  • Participate as a full member of the multidisciplinary team in assessing, planning, implementing, and evaluating individual treatment and rehabilitation plans
  • Liaise with relevant agencies, relatives, and carers with empathy
  • Provide information, support, and advice regarding treatment plans
  • Liaise with outside organisations for equipment provision, such as wheelchair services
  • Provide advice and support to staff by teaching relevant techniques to safely implement treatment plans

Requirements

  • HCPC registration
  • Experience utilizing an evidence-based, patient-centred approach to assess, plan, implement, and evaluate treatment plans
