Occupational Therapist (OT) - Home Care

Responsibilities

• Deliver age-appropriate occupational therapy to patients in their homes within the Greater Concord and Southern New Hampshire area
• Develop and implement appropriate plans of care (POC) and discharge goals in partnership with patients and caregivers
• Perform thorough assessments, interventions, and evaluations within the scope of practice defined by the NH Occupational Therapy Board
• Document accurate and ongoing assessments, treatments, and progress in accordance with agency timeframes
• Coordinate care with the interdisciplinary team and communicate effectively with ordering providers
• Supervise Certified Occupational Therapy Assistants assigned to patients
• Coach, mentor, and precept new occupational therapy employees and students

Requirements

• Graduate of an approved occupational therapy program
• Current New Hampshire occupational therapy license
• One year of occupational therapy experience
