Occupational Therapist (OT) - Home Care

Responsibilities

• Deliver age-appropriate home care through assessment, planning, intervention, implementation, and evaluation.
• Develop and implement appropriate plans of care (POC) and discharge goals in partnership with patients and caregivers.
• Document accurate and ongoing assessments, treatments, and progress in accordance with agency timeframes.
• Coordinate patient care with members of the interdisciplinary team and other healthcare professionals.
• Supervise Certified Occupational Therapy Assistants assigned to patients.
• Engage patients and caregivers in meaningful education related to the care plan.
• Coach, mentor, and precept new occupational therapy employees and students.

Requirements

• Graduate of an approved occupational therapy program.
• Current New Hampshire occupational therapy license.
• One year of occupational therapy experience.
