Occupational Therapist

Responsibilities

• Provide high-quality clinical services and technical skilled care within the scope of practice defined by state laws
• Deliver patient care in accordance with the established plan of care and in coordination with the healthcare team
• Document patient visits thoroughly and completely in the electronic medical record per regulatory and payer requirements
• Actively participate in the care coordination process, including reporting changes in patient condition and attending case conferences
• Communicate effectively with patients, families, physicians, and other clinicians to ensure seamless care delivery

Requirements

• Current and unrestricted LPN licensure in the state of practice
• Valid driver's license and vehicle insurance
• Access to a dependable vehicle or ability to use public transportation

Preferred Qualifications

• Current CPR certification or ability to obtain it within 90 days of hire
• Previous experience in home care settings
• Ability to manage multiple tasks simultaneously and work independently
• Strong communication, writing, and organizational skills
• Ability to work flexible hours to meet client needs
