About the role
Responsibilities
- Accept and treat patients within your designated geographic area
- Provide 1:1 treatment
- Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
- Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
- Educate family members, home health aides, and other caregivers on proper therapy techniques
- Plan and carry out occupational therapy programs to maintain, improve or restore physical function
- Perform and document an initial exam to identify problems and determine a diagnosis
- Provide timely and accurate documentation for each visit
- Evaluate effects of treatment and adjust treatments to achieve maximum benefit
- Treat geriatric patients
Requirements
- Occupational Therapist Program Graduate
- Licensed Occupational Therapist in the state of New York or eligible
- Ability to travel to patients’ homes within the assigned geographic territory
- Strong communication skills
