Responsibilities
- Deliver personalized therapy to patients in their homes
- Manage a manageable caseload with 1:1 care
- Maintain timely communication with patients, support staff, and management
- Complete streamlined documentation via EMR
Requirements
- Occupational Therapist Program Graduate
- Licensed Occupational Therapist in the state of New York or eligible
- Ability to travel to patients' homes within the assigned geographic territory
- Strong communication skills

- Home health experience
