About the role
Responsibilities
- Accept and treat patients within a designated geographic area
- Provide 1:1 treatment and evaluate patients to develop individualized treatment plans
- Deliver care aligned with professional standards and physician orders
- Educate family members and caregivers on proper therapy techniques
- Plan and carry out programs to improve physical function and relieve pain
- Perform and document initial exams and diagnoses
- Provide timely and accurate documentation for each visit
- Evaluate treatment effects and adjust plans to achieve maximum benefit
Requirements
- Occupational Therapist Program Graduate
- Licensed Occupational Therapist in the state of New York or eligible
- Ability to travel to patients' homes within the assigned territory
- Strong communication skills for interacting with patients and staff
- Willingness to treat geriatric patients
Preferred Qualifications
- Previous home health experience
