Occupational Therapist

Responsibilities

• Deliver strengths-based, person-centred OT assessments to support independence at home
• Provide practical support including equipment fitting, adaptations, demonstrations and assistive technology
• Offer information, advice and guidance to build self-care skills, strengthen networks and access community resources
• Support use of Connect to Support and collaborate with the Social Prescribing Service
• Build community connections to reduce isolation, prevent hospital admissions and support timely discharge
• Lead or contribute to case management, safeguarding reviews and best-interest decisions
• Work collaboratively across Adult Social Care, providing advice and training to colleagues
• Liaise with housing partners and surveyors to ensure adaptations meet assessed needs

Requirements

• HCPC-registered Occupational Therapist
• Strengths-based, solution-focused practitioner who values people as experts in their own lives
• Strong assessment skills and confidence delivering proportionate, person-centred interventions
• Experience with equipment, adaptations and assistive technology
• Ability to build relationships with individuals, families, communities and partner agencies
