About the role
Responsibilities
- Complete assessments, reviews, and care planning within the community
- Support individuals to maximise independence using strengths-based approaches
- Assess for equipment, adaptations, and moving & handling needs
- Work collaboratively within multidisciplinary locality teams
- Manage a varied caseload in a fast-paced environment
Requirements
- HCPC-registered Occupational Therapist
- Experience in Adult Social Care or community settings
- Strong assessment and clinical reasoning skills
- Ability to prioritise and manage workload effectively
