About the role
Responsibilities
- Conduct strengths-based reviews and develop support plans
- Perform Mental Capacity Act assessments
- Contribute to Multi-Disciplinary Continuing Health Care assessments
- Collaborate with the reviewing service team
- Carry out risk assessments and provide recommendations for assistive equipment and home adaptations
Requirements
- Qualified Occupational Therapist with HCPC registration
- Minimum 2 years of experience
- Enhanced DBS check
- Ability to travel as required
- Strong communication and organizational skills
- Proficiency in case management and report writing

- Experience with community settings and social care services
