About the role
Responsibilities
- Build effective relationships with residents, professionals, and community networks
- Conduct assessments using appropriate frameworks and develop cost-effective support plans
- Manage your own caseload independently, prioritising tasks and delivering high-quality outcomes
- Participate in planning and reviewing cases, contributing technical expertise and insight
- Provide feedback to support service improvement and operational planning
Requirements
- Qualified, HCPC-registered Occupational Therapist
- Experience working with adults with learning disabilities and transition work
- Knowledge of the Care Act 2014 and Mental Capacity Act 2005
- Full UK driving licence and regular access to a vehicle
- Strong decision-making and teamwork skills
- Ability to work under pressure
