About the role
Responsibilities
- Conduct community-based assessments to support independence and reduce care needs
- Deliver interventions and rehabilitation plans tailored to individual goals
- Collaborate with health and social care colleagues to ensure coordinated support
- Maintain accurate records in line with professional and statutory requirements
- Provide face-to-face visits each week as part of case management
Requirements
- HCPC Registration
- Relevant Occupational Therapy qualifications
- Experience in community-based roles, ideally within Adult Social Care
- Strong communication, organisational, and assessment skills
- Ability to travel across Bexhill, Hastings, and Rother
