About the role
Responsibilities
- Manage a varied caseload of hospital discharges and community referrals
- Complete risk-assessed home visits and provide clinical oversight for reablement plans
- Direct and support Reablement Officers delivering care programmes
- Work collaboratively with a multidisciplinary team to achieve positive outcomes
Requirements
- HCPC registered Occupational Therapist
- Minimum 2 years post-qualification experience (Band 6 level or equivalent)
- Proven experience in case work and physical rehabilitation
- London-based for urgent case management
- Ability to work flexibly (Monday–Friday)
