Occupational Therapist

Responsibilities

• Conduct in-home assessments and functional evaluations
• Deliver personalized, evidence-based recommendations for home modifications
• Educate clients and caregivers on fall prevention and safe mobility techniques
• Engage clients and families in collaborative care planning
• Work closely with the Director of Clinical Operations to share findings and support process improvement
• Maintain accurate and timely documentation through electronic systems

Requirements

• Degree in Occupational Therapy from an accredited program
• Active, unencumbered OT license
• 2+ years of clinical experience
• Reliable transportation and valid driver’s license
• Proficiency with technology and digital documentation
• Ability to work independently while collaborating with a remote team

Preferred Qualifications

• Experience in home health, aging in place, or disability services
• Experience with home modification work
