Responsibilities
- Perform thorough pediatric and adolescent assessments to determine patient needs
- Implement appropriate treatment plans using evidence-based practice
- Communicate treatment plans effectively with patients and caregivers, involving them in goal setting and intervention processes
- Monitor patient progress and adjust treatment plans as necessary
- Determine appropriate discharge timelines based on patient outcomes
- Collaborate with other healthcare professionals within the organization and the community to ensure coordinated care
- Participate actively in team meetings and complete all mandatory assignments
