About the Company

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care by utilizing technology to support field clinicians and reduce administrative friction.

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits
• Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence
• Recommend and train on adaptive equipment and home modifications as needed
• Coordinate with nursing and the broader care team when condition or needs change
• Document promptly and thoroughly in Homecare Homebase (HCHB)

Requirements

• Active Texas OT license in good standing
• Strong clinical judgment and comfort working independently in the field
• Current driver’s license and comfort with daily travel across assigned territory
• Ability to carry and transport basic therapy equipment
• Competency in core home health OT scope including ADL/IADL training and home safety

Preferred Qualifications

• Prior home health experience
• Homecare Homebase (HCHB) or similar home health EMR experience
• Experience with high-signal OT populations such as neuro, CVA, fall risk, or dementia
