About the role
Responsibilities
- Complete in-home OT evaluations and deliver skilled OT visits
- Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence
- Recommend and train on adaptive equipment and home modifications as needed
- Coordinate with nursing and the broader care team when condition or needs change
- Document promptly and thoroughly in Homecare Homebase (HCHB)
Requirements
- Active Texas OT license in good standing
- Strong clinical judgment and comfort working independently in the field
- Current driver’s license and comfort with daily travel across assigned territory
- Ability to carry and transport basic therapy equipment
- Competency in core home health OT scope
Preferred Qualifications
- Prior home health experience
- Homecare Homebase (HCHB) or similar home health EMR experience
- Experience with high-signal OT populations such as neuro, CVA, fall risk, or dementia
- Strong organization and time management skills
