About the role
Responsibilities
- Complete in-home OT evaluations and deliver skilled OT visits
- Build and execute plans of care focused on ADLs/IADLs, safety, and functional independence
- Recommend and train on adaptive equipment and home modifications
- Coordinate with nursing and the broader care team regarding changes in patient condition
- Document promptly and thoroughly in Homecare Homebase (HCHB)
Requirements
- Active Michigan OT license in good standing
- Strong clinical judgment and ability to work independently in the field
- Valid driver’s license, auto insurance, and reliable transportation for daily travel
- Ability to carry and transport basic therapy equipment
- Competency in core home health OT scope including ADL/IADL training and home safety
Preferred Qualifications
- Prior home health experience
- Experience with Homecare Homebase (HCHB) or similar home health EMR
- Experience with high-signal OT populations such as neuro, CVA, fall risk, or dementia
