Occupational Therapist

Responsibilities

• Complete in-home OT evaluations and deliver skilled OT visits
• Build and execute plans of care focused on ADLs/IADLs, safety, and functional independence
• Recommend and train on adaptive equipment and home modifications
• Coordinate with nursing and the broader care team regarding changes in patient condition
• Document promptly and thoroughly in Homecare Homebase (HCHB)

Requirements

• Active Michigan OT license in good standing
• Strong clinical judgment and ability to work independently in the field
• Valid driver’s license, auto insurance, and reliable transportation for daily travel
• Ability to carry and transport basic therapy equipment
• Competency in core home health OT scope including ADL/IADL training and home safety

Preferred Qualifications

• Prior home health experience
• Experience with Homecare Homebase (HCHB) or similar home health EMR
• Experience with high-signal OT populations such as neuro, CVA, fall risk, or dementia
