Nursing Care Coordinator

Responsibilities

• Administer total patient care for individuals with tuberculosis and their families based on professional nursing standards.
• Assess patient health status and implement individualized care plans in accordance with medical plans of care.
• Provide direct patient care and supervise other health team members to ensure proper implementation of care plans.
• Educate patients and significant others regarding health status, illnesses, and discharge/home maintenance.
• Collaborate with the Medical Director, Nurse Manager, and public health departments to participate in TB training and research.
• Perform telephone triage to accurately assess and provide safe advice for urgent and non-urgent patient calls.

Requirements

• Registered Nurse (RN) licensure in the State of New Jersey in good standing.
• At least three (3) years of outpatient experience.
• At least one (1) year of clinical nursing experience.
• Familiarity with Electronic Medical Records (EMR).
• Current ACLS and BCLS certifications.
• Demonstrated ability to provide culturally competent health education.

Preferred Qualifications

• Bachelor of Science in Nursing (BSN).
