About the role

Responsibilities

  • Conduct comprehensive clinical assessments, including medical history reviews, medication reconciliation, vital signs, and physical exams
  • Perform evidence-based screenings and point-of-care testing as appropriate
  • Support members during short-term sub-acute rehabilitation episodes
  • Ensure accurate and complete clinical documentation, including ICD-10 coding
  • Collaborate with interdisciplinary care teams to coordinate member care
  • Lead advanced illness and advanced care planning conversations
  • Provide essential counseling and education to patients and their caregivers

Requirements

  • For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
  • For PAs: Current NCCPA certification and state licensure
  • Active, unrestricted licensure in the state of practice without disciplinary actions
  • Active and unrestricted driver's license and access to reliable transportation
  • Current, active DEA licensure/prescriptive authority (or ability to obtain post-hire)
  • Ability to navigate varied environments and obtain a collaborative practice agreement if required by state law

Preferred Qualifications

  • 1+ years of clinical experience as an Advanced Practice Clinician (long-term care setting preferred)
  • Experience in geriatric medicine, long-term care, senior living, or home care settings
  • Experience managing patients with complex behavioral, social, or functional needs
  • Strong understanding of Geriatrics, Chronic Illness, and end-of-life discussions
  • Proficiency with electronic medical records (EMR) and healthcare technology
