Nest is the first value-based care provider built for families, on a mission to make comprehensive medical, behavioral, and social care radically accessible to America’s highest risk families. Founded in 2022, Nest utilizes a family-based, in-home, and virtual care model to provide primary adult and pediatric care, mental health support, and social services.

Responsibilities

• Provide in-person primary care visits as part of a comprehensive longitudinal care model
• Conduct adult and pediatric wellness visits to address quality care gaps
• Provide acute and chronic care requiring physical examination or diagnostic testing
• Reconcile and prescribe medications, order labs, and refer to specialist care
• Partner with an interdisciplinary team including MDs, LCSWs, RNs, and psychiatrists
• Document care and maintain comprehensive care plans
• Help families identify and access health and social care resources
• Participate in community outreach efforts

Requirements

• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• Active, unrestricted Arizona license
• Current BLS/CPR certification
• Unrestricted driver’s license and reliable personal vehicle
• Ability to work at least 1-2 days per week (8-10 hours per day)
• Ability to transport healthcare supplies up to 25 lbs.

Preferred Qualifications

• At least 2 years of experience providing clinical services to pediatric and adult patients
• Experience with chronic medical and behavioral health conditions
• Leadership experience in an interdisciplinary care team
