Responsibilities

• Provide comprehensive primary care for acute and chronic conditions
• Efficiently manage 23–24 patients per day using 20-minute visit templates
• Actively address care gaps (HEDIS) and ensure accurate HCC/RAF documentation
• Direct Medical Assistants and Scribes to prep charts and pend orders
• Maintain chart hygiene with 95% of charts signed and closed within 48 hours
• Follow visit templates to maintain steady patient flow

Requirements

• Current APRN or PA-C license in the state of practice
• Minimum 3 years post-graduate experience in Primary Care, Family Medicine, or Internal Medicine
• Experience in high-volume environments (23+ patients/day)

Preferred Qualifications

• Prior experience in value-based care (HEDIS, RAF/HCC coding)
• Experience with Athena EMR

• Base salary up to $150,000
