Nurse Practitioner/Physician Assistant

Responsibilities

• Evaluate patients' acute and chronic conditions
• Manage multiple medications, mental health, and social-emotional needs
• Educate patients in a thorough, professional, and empathetic manner
• Partner with care team members in assessing, planning, and providing patient care
• Float to various clinic locations including Highland Park, South Minneapolis, Eagan, Rosedale, and Crystal as needed

Requirements

• Masters degree in Nursing (required) or Masters degree in Physician Assistant (required)
• Adult or Family Nurse Practitioner or Physician Assistant with experience in Geriatrics
• Completion of 2080 hours of post-graduate collaborative practice hours in an integrated clinical care setting
• Registered Nurse Licensure or valid PA License in the state of Minnesota
• Current Basic Life Support (BLS) certification from AHA
• Eligibility for DEA License and NPI number
• Valid Minnesota Driver’s License

Preferred Qualifications

• Doctor of Nursing Practice (DNP)
• Experience in Primary Care, Palliative Care, or Family Medicine
• Advanced Life Support (ALS) certification
• Knowledge of population health and quality metrics
