Nurse Practitioner

Responsibilities

• Provide care in nursing and long-term care facilities
• Manage chronic conditions and perform full assessments
• Close care gaps (HEDIS and STAR measures)
• Lead care planning with facility teams
• Support and educate patients’ families

Requirements

• Active, unrestricted APN license in Michigan
• Board certified and Medicare credentialed
• 6+ months in skilled nursing or long-term care
• Proficiency with digital tools and team-based care
• Ability to work onsite in Saginaw, MI

Benefits

• Base pay of $123,000–$135,000 plus corporate bonus
