About the role

Responsibilities

  • Provide specialized primary care to adults with intellectual and developmental disabilities (IDD), including autism, cerebral palsy, and Down syndrome
  • Deliver care through a hybrid model: 60% in community settings via home visits and 40% on-site at the Freeman Center
  • Collaborate with interdisciplinary teams, caregivers, families, and specialists to coordinate comprehensive patient care
  • Document clinical findings, treatment plans, and actions in accordance with departmental standards
  • Participate in continuous quality improvement efforts and provide education to other healthcare professionals
  • Maintain accurate medical records and stay current on relevant medical and legal issues impacting patient care

Requirements

  • Master’s Degree in Nursing from an accredited program
  • Active Ohio APRN license with prescriptive authority
  • Active Board Certification in an appropriate nursing specialty
  • Three years of nursing experience

Preferred Qualifications

  • Previous experience working with adults with intellectual and developmental disabilities (IDD)
