About the role
Responsibilities
- Serve as the Most Responsible Provider (MRPr) for a diverse panel of patients, delivering comprehensive primary care across the lifespan
- Conduct advanced health assessments, diagnose acute and chronic conditions, and manage patient care plans
- Prescribe medications, order diagnostic tests, and implement strategies for health promotion and illness prevention
- Work collaboratively within an interdisciplinary team including physicians, nurses, social workers, and pharmacists
- Provide coverage for the Virtual Emergency Department via video visits for urgent, non-life-threatening issues
- Act as a leadership change agent by coaching and training Family Health Team Registered Nurses
- Support and engage in team-based research and professional development activities
Requirements
- Master's Degree in Nursing from a recognized program
- Primary Care Nurse Practitioner Certificate (or currently in progress)
- Current registration with the College of Nurses of Ontario (CNO) as an RN (EC)
- Current Basic Cardiac Life Support (BCLS) certification
- Minimum of 5 years of nursing experience, including at least 1 year in an advanced role
- Strong interpersonal, communication, and organizational skills
- Ability to utilize information technology and manage complex workloads
