About the role

Responsibilities

  • Serve as the Most Responsible Provider (MRPr) for a diverse panel of patients, delivering comprehensive primary care across the lifespan
  • Conduct advanced health assessments, diagnose acute and chronic conditions, and manage patient care plans
  • Prescribe medications, order diagnostic tests, and implement strategies for health promotion and illness prevention
  • Work collaboratively within an interdisciplinary team including physicians, nurses, social workers, and pharmacists
  • Provide coverage for the Virtual Emergency Department via video visits for urgent, non-life-threatening issues
  • Act as a leadership change agent by coaching and training Family Health Team Registered Nurses
  • Support and engage in team-based research and professional development activities

Requirements

  • Master's Degree in Nursing from a recognized program
  • Primary Care Nurse Practitioner Certificate (or currently in progress)
  • Current registration with the College of Nurses of Ontario (CNO) as an RN (EC)
  • Current Basic Cardiac Life Support (BCLS) certification
  • Minimum of 5 years of nursing experience, including at least 1 year in an advanced role
  • Strong interpersonal, communication, and organizational skills
  • Ability to utilize information technology and manage complex workloads
