About the role

Responsibilities

  • Provide autonomous primary health care to long-term care residents within the legislated NP scope of practice
  • Diagnose, prescribe, order and interpret lab tests, x-rays, and ultrasounds per regulations
  • Collaborate with the Medical Director, physicians, and multidisciplinary teams on resident care plans
  • Consult and refer to other health care professionals and community services as needed
  • Provide medical coverage upon request of Medical Directors and physician staff
  • Educate staff, residents, and families on clinical issues, advanced care planning, and treatment options
  • Develop, implement, and evaluate clinical policies, practice guidelines, and nursing department initiatives
  • Participate in management meetings, Professional Practice Committees, and Pharmacy and Therapeutics Committees
  • Liaise with community agencies to ensure appropriate utilization of specialized services
  • Maintain compliance with the Ontario Occupational Health and Safety Act and organizational policies

Requirements

  • Current registration with the College of Nurses of Ontario as an RN(EC) / Nurse Practitioner (unrestricted)
  • Demonstrated expertise in geriatric care and adult chronic disease management
  • Excellent communication, leadership, problem-solving, and interpersonal skills
  • Effective program development, evaluation, resource allocation, and project management abilities
  • Proven ability to manage interprofessional teams and build positive working relationships
  • Self-directed with the ability to work independently and achieve outcomes
  • Excellent report writing skills
  • Valid driver's license and vehicle required
