About the role
Responsibilities
- Provide team-based clinical care in a medical home setting
- Perform routine clinic operations, including taking patient histories and making physical assessments
- Support patients and their families in self-management, self-efficacy, and behavior change
- Adhere to the mission of providing quality, integrated healthcare to the community
Requirements
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Unrestricted Physician Assistant or Nurse Practitioner license in good standing in the State of Colorado
- Demonstrated sensitivity to low-income and ethnic minority communities
Preferred Qualifications
- 3 years of experience in a clinical setting as an NP or PA
- Fluency in Spanish
Benefits
- Competitive annual salary ranging from $58,333 to $77,083
- $5,000 sign-on bonus
- Part-time schedule (2.5 days per week)
