Nurse Practitioner/Physician Assistant

Responsibilities

• Provide team-based clinical care to patients and their families in a medical home setting
• Perform routine clinic operations, including taking patient histories and making physical assessments
• Support patients in self-management, self-efficacy, and behavior change
• Adhere to the mission of providing quality, integrated healthcare to the community

Requirements

• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• Unrestricted Physician Assistant or Nurse Practitioner license in the State of Colorado
• Sensitivity to low-income and ethnic minority communities
• 3 years of experience in a clinical setting preferred
• Spanish fluency is strongly preferred
