For over 125 years, Calvary Hospital and Calvary @Home located in the Bronx, New York has been dedicated to the palliative care of advanced cancer and other terminally ill patients and their families. Through a caring, compassionate, philosophy of non-abandonment, the hospice provides a full spectrum of palliative services, including emotional, spiritual, and physical care.

Responsibilities

• Conduct comprehensive patient assessments, focusing on physical, psychosocial, and spiritual needs
• Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team
• Facilitate advanced care planning and goals-of-care discussions
• Prescribe medications, treatments, and interventions consistent with licensure and scope of practice
• Provide clinical leadership and mentoring to the interdisciplinary team
• Ensure compliance with Joint Commission, state, and federal regulations
• Participate in quality improvement initiatives and program evaluation
• Travel to patient locations throughout the New York metro area

Requirements

• Current NYS Registered Nurse license
• Current NYS Nurse Practitioners license
• Master’s or Doctoral degree in Nursing
• Minimum of two years of clinical experience in palliative care, hospice, oncology, or geriatrics preferred
• New York State Driver’s License preferred
• Expertise in complex symptom management and advance care planning
