Nurse Practitioner or Physician Assistant

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plan members
• Perform medical history reviews, medication reconciliation, vital signs, and physical exams
• Identify and document diagnoses to support care management and treatment planning
• Communicate clinical findings to members' primary care providers to address gaps in care
• Educate members on disease processes, medications, and treatment compliance
• Address social determinants of health and provide necessary referrals

Requirements

• For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
• For PAs: Current NCCPA certification and state licensure
• Active, unrestricted licensure in the state of practice
• Active and unrestricted driver's license
• Access to reliable transportation for home visits
• Ability to navigate varied home environments and perform physical assessments (sitting, standing, kneeling)

Preferred Qualifications

• 1+ years of clinical experience, specifically in family, geriatric, or home health settings
• Proficiency with electronic medical records (EMR) and healthcare technology
• Proven communication skills with geriatric or Medicare populations
• Ability to transport medical equipment weighing up to 30 pounds
