About the role
Responsibilities
- Conduct comprehensive in-home health assessments, including medical history review, medication reconciliation, vital signs, and physical exams
- Identify and document diagnoses to support care management and treatment planning
- Communicate clinical findings to members' Primary Care Providers (PCP) to address gaps in care
- Educate members on disease processes, medications, and treatment compliance
- Recognize and appropriately intervene in urgent or emergent situations
- Address social determinants of health and provide necessary referrals
Requirements
- For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- For PAs: Current NCCPA certification and state licensure
- Active, unrestricted licensure in the state of practice (or ability to obtain by start date)
- Active and unrestricted driver's license with access to reliable transportation for home visits
- Ability to navigate varied home environments and perform physical assessments (sitting, standing, kneeling)
Preferred Qualifications
- 1+ years of clinical experience, specifically in family, geriatric, or home health settings
- Proficiency with electronic medical records (EMR) and healthcare technology
- Proven communication skills with geriatric or Medicare populations
- Ability to transport equipment weighing up to 30 pounds and navigate stairs
